WHAT DO I NEED TO BRING OR HAVE READY FOR MY FIRST APPOINTMENT? WHAT IS AN APPOINTMENT LIKE?

On your first in-person visit, we will need your ID and paperwork. We encourage clients to send it prior to their appointment electronically, but you may fill it out in person. An associate will then take your height, weight, and blood pressure (no blood work). We'll then discuss the medication Dr. Collado will prescribe, the legality & side effects of the medication, a nutrition plan, and recommended cardio regimen. Lastly, you'll meet with one of our providers, who will also send your prescription electronically to a pharmacy.

Virtual Appointment - Important Telemedicine Information:

  1. A state or federal ID must be presented at each appointment.

  2. New clients must submit their new client paperwork electronically prior to the appointment or the appointment cannot take place.

  3. Your weight loss associate will ask for your weight, meaning you will need to have access to a scale.

  4. Your weight loss associate will also ask for your blood pressure reading, meaning you will need to have access to a blood pressure machine. You can get this by either purchasing a blood pressure cuff or going to your nearest Walmart in the pharmacy area.

  5. We're currently unable to utilize pharmacies at Walmart, SamsClub, or CVS.

  6. We will be taking debit or credit card payments over the phone using Square.

  7. We are only able to assist Oklahoma and Witchita Falls, TX areas.

WHAT IS THE MINIMUM BODY MASS INDEX (BMI) THAT I CAN START OR RESTART THE PROGRAM?

We require a Body Mass Index (BMI) of 27.0 or higher which we measure upon your first visit or restarting visit. Check your BMI here. Acceptance and reacceptance into the program are at the sole discretion of the physicians.

WHAT ARE YOUR FEES?

This is not a contract or subscription service.

  • New Clients are $55

  • Follow-Up & Restarting Clients are $45

We also offer monthly B12 injections for $10.

A 30-day prescription is often between $12 and $30, depending on the pharmacy. We also encourage the use of GoodRx.com, and we do not dispense medications.

WHAT KIND OF PAYMENT IS ACCEPTED AND DO YOU TAKE INSURANCE?

We accept all credit cards and cash, but no checks. We do not file under insurance.
For virtual visits, payments are done via Square Payment Systems where you'll read a card number so it can be processed manually. We will only save your card information at your request, never without your permission.

CAN I RESTART THE PROGRAM?

If it has been 8 weeks (56 days) or longer since we've seen you, then you have the opportunity to restart, but at the physicians' discretion.

ARE THERE OTHER OPTIONS BESIDES PHENTERMINE?

Yes, and the appointment price stays the same. Please note, we cannot allow individuals to join the program if there are factors that would prevent them from joining regularly (such as those listed under the eligibility criteria).
